Was thinking Ada; perhaps just have it as kg.com/survey - I know its stuff all difference, but its clean looking and simple, personally i hate having to type the extensions, just a thought.

okay just to clarify...this is a "work together" project, not a challenge to have a script done first. The challenge is getting it done with the security and controls that we need to make this a statistically valid survey in the eyes of government, in as short of a time space as possible. I'd like to start sending cards to school with Michael on Monday morning. Could be a problem with that though, Shane....more than one user in the same household. Jackie & Mike both did the survey from the office computer. (Mike our driver not Michael)

We give them a visitor number, they write that down & keep it. We ask them for their first name only; by cross-referencing the number they bring in with the number in the database we can verify their first name, making sure it was that person who did the survey. In Canada, surveys don't *have* to be anonymous, and all surveys which offer prizes ask for some type of identity confirmation.
